Bock Studio ACTIVITY PERMISSION SLIP I give permission for MaryEllen Bock to deduct the appropriate music fees*
so that ___________________________may enroll to participate in those events. Enter student name
NON REFUNDABLE: I understand that these fees are non-refundable once they have been paid by Ms. Bock to the event chair. RESPONSIBILITY: I understand it is my responsibility to check our home calendars to be sure that my student can commit to his or her attendance at each event entered. I understand that I must clear the entire day or weekend since dates and times for events are not assigned until the week before each event. ILLNESS: I understand that if my student does not participate for any reason, including illness, the moneys sent are forfeited.
